Hey, just wanted to let everyone know that our forms for eligibility are underneath the ” Donation Forms” tab. The document is a PDF, so all that has to be done is click in the boxes, fill out the form, and e-mail it back to Elliot.Hood@gmail.com with some contact info so we can follow-up with you. Soon, we will have the photo release form so we can publish some of the guys with their new shoes!
